For college-bound students and families

Prepare before the send-off.

The Launch Packet helps families organize important information, prepare better questions, and coordinate practical next steps before a student leaves for college.

The reason to prepare

Fewer unanswered questions when time matters.

“After more than three decades in emergency response, I’ve seen how hard it is for families to make decisions under pressure. Preparation means fewer unanswered questions when time matters.”

— Michael Bollentin

When a student becomes a legal adult, being their parent no longer guarantees access to medical or education information. Some emergency disclosures are permitted, but families should discuss access, information sharing, and decision-making before a crisis.

Rules, forms, and signing requirements vary by state, school, and healthcare provider. The student participates in the process and chooses whom to authorize and what to share.

Authoritative background: medical information and family access · student education-record rights.

What True Meridian does

We help gather school and provider instructions, organize existing documents and contact information, record the student’s stated priorities, and prepare focused questions for an attorney. When signing support is needed, we can coordinate a New Jersey notary appointment and appropriate witnesses based on instructions supplied by counsel or the receiving institution.

What the family receives

An organized preparation packet showing the information already available, questions that need professional answers, and the next steps still requiring attention. Completed legal documents depend on the student’s decisions and any necessary work by a licensed attorney.

Common topics to discuss

  • HIPAA authorization
  • Healthcare power of attorney or medical proxy
  • FERPA release or the school’s information-sharing process
  • Financial power of attorney, including whether durable authority is appropriate

These are discussion topics—not a required document list. A licensed attorney advises which legal instruments fit the student’s circumstances.

How it works

A practical, student-centered process.

The student remains involved in the decisions and controls what information may be shared.
